If you would like to schedule an appointment with one of our practitioners for an orthotic or prosthetic consultation, or if you are a vendor and would like to schedule an appointment, please complete the Contact Us form. 

Once we receive your appointment request via email, we will gladly call you at your home or office to schedule an appointment or schedule a meeting.

For patients: once your appointment is scheduled, you may complete the patient information form, which will inform you of your privacy rights and OPE facility practices.
